1. Payment is due within fourteen (14) calendar days of the date of this Quotation Estimate or upon receipt of the invoice, whichever is earlier. Payment may be made via Electronic Funds Transfer (EFT) to the practice bank account or by debit order with the client's written consent.
2. A deposit of 30% of the total Quotation Estimate amount (R 8,262.75) is required to secure the Occupational Therapist's schedule and confirm the commencement date of services in Johannesburg, South Africa.
3. The remaining balance shall be invoiced in two equal instalments: 50% at the midpoint of the treatment programme (after session six) and 50% upon completion of the final session and submission of the discharge report.
4. Late payments will attract interest at the rate of 2% per month or the maximum rate permitted by the National Credit Act of South Africa, whichever is lower.
5. This Quotation Estimate does not constitute a binding contract until both parties have signed and returned the acceptance section below. The Occupational Therapist reserves the right to decline or modify services if the client's condition changes significantly after the initial assessment.
6. All clinical records and reports generated by the Occupational Therapist remain the property of the client and may be shared with the client's General Practitioner, medical aid scheme, or other healthcare providers upon written request.
7. Cancellations of scheduled sessions must be communicated at least twenty-four (24) hours in advance. Sessions cancelled with less than twenty-four hours' notice will be charged at 50% of the session fee.
8. This Quotation Estimate is governed by the laws of the Republic of South Africa. Any disputes arising from this engagement shall be subject to the jurisdiction of the courts in Johannesburg, South Africa.
