1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, prices may be subject to revision based on current market rates in Spain Madrid for Occupational Therapist services.
2. All services will be performed by a registered and licensed Occupational Therapist holding a valid professional registration with the Madrid College of Occupational Therapists (Colegio Oficial de Terapeutas Ocupacionales de Madrid).
3. The estimated total of €3,859.90 covers a three-month treatment programme. Any extension beyond this period will require a revised Quotation Estimate issued by our clinical management team.
4. Payment terms: 50% deposit due upon acceptance of this Quotation Estimate; the remaining 50% to be settled in two equal monthly instalments. Payment may be made via bank transfer (IBAN provided upon confirmation), credit card, or direct debit in accordance with Spanish consumer protection law (Ley General para la Defensa de los Consumidores y Usuarios).
5. Cancellation policy: Sessions cancelled with less than 24 hours' notice will be charged at 50% of the session fee. Home visits within the Spain Madrid metropolitan area cancelled with less than 48 hours' notice will be charged in full.
6. All clinical records and treatment documentation will be stored in compliance with the Spanish Organic Law 3/2018 on Personal Data Protection and the General Data Protection Regulation (GDPR). Client data will not be shared with third parties without explicit written consent.
7. This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties. Acceptance may be communicated via signed copy of this document or written email confirmation to the address listed above.
8. In the event that the client holds a private health insurance policy (seguro privado) that covers Occupational Therapist services in Spain Madrid, our billing department will assist in submitting the relevant documentation for reimbursement. The client remains responsible for any co-payment or deductible amounts not covered by the insurer.
9. The Occupational Therapist assigned to this case will provide a written progress report at the end of each month, detailing functional gains, areas requiring continued intervention, and recommendations for the subsequent treatment phase.
10. This Quotation Estimate is governed by the laws of the Kingdom of Spain. Any disputes arising from the interpretation or execution of this document shall be subject to the jurisdiction of the competent courts in Madrid, Spain.
