Our qualified Occupational Therapist will provide the following services tailored to the client's specific rehabilitation and functional independence needs:
- Initial Comprehensive Assessment: A full functional evaluation conducted at the client's residence in Sri Lanka Colombo, including ADL (Activities of Daily Living) analysis, cognitive screening, and environmental assessment.
- Individualized Treatment Planning: Development of a personalized rehabilitation program targeting upper limb function, fine motor skills, sensory integration, and daily living independence.
- Weekly Therapy Sessions: One-on-one sessions with a licensed Occupational Therapist, conducted either at our Colombo 03 clinic or at the client's home within Sri Lanka Colombo.
- Home Modification Consultation: Assessment and recommendation of ergonomic adaptations, assistive devices, and environmental modifications to promote safe and independent living.
- Caregiver Training: Instruction and guidance for family members and caregivers on therapeutic exercises and daily management strategies.
- Progress Monitoring and Reporting: Monthly progress reports and quarterly review meetings to adjust the treatment plan as needed.

Important Terms Governing This Quotation Estimate
- This Quotation Estimate is valid for a period of thirty (30) days from the date of issue. After this period, rates may be subject to revision due to market conditions in Sri Lanka Colombo.
- A minimum deposit of 30% of the total amount (LKR 170,982) is required to confirm the engagement and schedule the initial Occupational Therapist assessment.
- Remaining payments shall be made in monthly installments aligned with the 12-week therapy program, due on the 5th of each month.
- All Occupational Therapist sessions must be booked at least 48 hours in advance. Cancellations made less than 24 hours prior to a scheduled session will incur a 50% session fee.
- Services will be delivered by a registered Occupational Therapist holding a valid license from the Sri Lanka Nursing and Midwifery Council or the relevant professional regulatory body.
- Home visit services are limited to the Sri Lanka Colombo municipal and suburban boundaries. Travel to locations outside this area will be quoted separately.
- WellSpring Occupational Therapy Services (Pvt) Ltd reserves the right to modify the treatment plan based on clinical progress, and any additional sessions beyond the 12-week program will be billed at the standard weekly rate.
- All client information and therapy records will be maintained in strict confidence in accordance with Sri Lankan data protection regulations.
- This Quotation Estimate does not constitute a medical diagnosis or guarantee of specific therapeutic outcomes. The Occupational Therapist will provide best-practice care based on current clinical evidence.
