Note: All costs are quoted in Tanzanian Shillings (TZS) and are inclusive of the Occupational Therapist's professional fees, travel within Dar es Salaam, and standard therapeutic materials. Costs for specialised imported equipment or services outside the Dar es Salaam municipal boundary will be quoted separately. This Quotation Estimate is subject to revision if the scope of services is materially altered by mutual agreement. Terms and Conditions

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, pricing may be subject to revision based on prevailing market conditions in Tanzania Dar es Salaam.
Payment terms: A 40% advance payment is due upon acceptance of this Quotation Estimate. The remaining 60% shall be paid in equal monthly instalments over the twelve-month service period.
All payments shall be made in Tanzanian Shillings (TZS) via bank transfer to the account details provided by Rehabilitation & Wellness Care Centre, Dar es Salaam.
The Occupational Therapist assigned to this engagement shall hold a valid licence issued by the Tanzania Health Professions Council and shall maintain professional indemnity insurance throughout the contract period.
Either party may terminate this agreement with thirty (30) days' written notice. In the event of early termination, the client shall be liable for all services rendered up to the date of termination.
All patient records, assessment reports, and progress documentation generated by the Occupational Therapist shall remain the property of the client and shall be handled in strict accordance with the Tanzania Data Protection Act, 2022.
This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by an authorised representative.
Any disputes arising from this engagement shall be resolved through amicable negotiation, failing which they shall be referred to the competent courts in Dar es Salaam, Tanzania.

Acceptance

By signing below, the client acknowledges and accepts the terms, conditions, and pricing outlined in this Quotation Estimate for the Occupational Therapist services to be delivered in Tanzania Dar es Salaam.
