The Occupational Therapist will design and implement a personalized treatment plan targeting upper-limb function restoration, fine motor skill development, adaptive equipment training, and community reintegration. All sessions will be conducted at our fully equipped clinic in central Thailand Bangkok or at the client's private residence within the Bangkok metropolitan district, as specified in the engagement agreement.

3.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all rates listed herein are subject to revision based on current Occupational Therapist market rates in Thailand Bangkok.
3.2 The total estimated cost of THB 184,575 (one hundred eighty-four thousand five hundred seventy-five Thai Baht) is inclusive of 7% Value Added Tax as mandated by the Revenue Department of Thailand.
3.3 Payment terms: 50% deposit (THB 92,287.50) due upon acceptance of this Quotation Estimate; the remaining 50% (THB 92,287.50) payable upon completion of the full twelve-week Occupational Therapist program. Payment may be made via bank transfer to Siam Wellness Rehabilitation Center, Siam Commercial Bank, Account No. 012-3-45678-9, Bangkok branch.
3.4 All Occupational Therapist sessions are subject to a 24-hour cancellation notice. Cancellations made within 24 hours will be charged at 50% of the session fee.
3.5 Home-based Occupational Therapist visits within the Bangkok metropolitan area (BMA) are included at no additional travel surcharge. Visits outside the BMA boundary will incur a travel fee of THB 500 per trip.
3.6 The Occupational Therapist reserves the right to modify the treatment plan based on clinical progress. Any additional sessions beyond the scope outlined in this Quotation Estimate will be billed at the standard per-session rates listed above.
3.7 All patient records, assessment data, and progress reports generated by the Occupational Therapist are confidential and protected under the Personal Data Protection Act (PDPA) of Thailand, B.E. 2562 (2019).
3.8 This Quotation Estimate does not constitute a binding contract until countersigned by both parties. The formal Service Agreement will be executed upon acceptance.
