1. This Quotation Estimate is valid for a period of ninety (90) calendar days from the date of issue. Any acceptance beyond this period requires written reconfirmation from Kabul Vision Care Center.
2. Payment shall be made in US Dollars via bank transfer to the designated account of the service provider. Invoices are issued monthly, and payment is due within thirty (30) days of invoice receipt.
3. All Ophthalmologist personnel assigned to this contract in Afghanistan Kabul must present valid medical licenses, professional indemnity insurance, and security clearance documentation prior to commencing duties.
4. The service provider shall maintain a minimum of 95% clinical availability. In the event of an Ophthalmologist's absence exceeding five (5) consecutive working days, a qualified substitute shall be deployed within seventy-two (72) hours at no additional cost.
5. All patient records, diagnostic imaging, and clinical data generated under this Quotation Estimate shall be stored in accordance with Afghanistan's national health data protection regulations and the Kabul Ministry of Public Health data governance policy.
6. Force majeure events, including but not limited to security disruptions in Afghanistan Kabul, shall be addressed through mutual consultation. Neither party shall be liable for delays directly caused by such events.
7. This Quotation Estimate does not constitute a binding contract until countersigned by both parties and approved by the relevant procurement authority in Kabul.
8. Any amendments to the scope of Ophthalmologist services, patient volume, or service locations within Afghanistan Kabul must be documented in a formal change order signed by both parties.
