The Ophthalmologist engaged under this Quotation Estimate shall provide the following professional services at the client's facility located in China Guangzhou:

• Comprehensive eye examinations including visual acuity testing, refraction, intraocular pressure measurement, and retinal assessment.
• Diagnosis and management of common ophthalmic conditions including myopia, hyperopia, astigmatism, glaucoma, cataracts, and diabetic retinopathy.
• Prescription of corrective lenses, contact lenses, and topical or systemic ophthalmic medications.
• Pre-operative and post-operative care for minor ophthalmic surgical procedures.
• Patient education and follow-up consultations within the China Guangzhou metropolitan area.
• Collaboration with the in-house optometry team and referral coordination with tertiary hospitals in Guangzhou when specialized intervention is required.

The Ophthalmologist proposed for this engagement shall meet the following minimum qualifications as recognized by the National Health Commission of the People's Republic of China:

• Valid practicing license issued by the Guangdong Provincial Health Administration for the China Guangzhou region.
• Board certification in Ophthalmology from a recognized Chinese medical institution (e.g., Sun Yat-sen University Medical School, Southern Medical University).
• Minimum of eight (8) years of post-graduate clinical experience in ophthalmic practice.

• Late payments shall incur a penalty of 0.05% per calendar day on the outstanding balance.

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Should the client wish to proceed beyond this validity window, a revised Quotation Estimate reflecting any market adjustments in China Guangzhou shall be issued. Acceptance of this Quotation Estimate constitutes a binding agreement between both parties, subject to the terms outlined herein and applicable Chinese healthcare regulations.

1. This Quotation Estimate does not constitute a final invoice. Final billing shall be adjusted based on actual service utilization by the Ophthalmologist during the contract period.
2. The Ophthalmologist's working hours, leave entitlements, and performance evaluations shall be governed by the separate Service Level Agreement (SLA) to be executed concurrently with this Quotation Estimate.
3. Either party may terminate the engagement with a written notice of sixty (60) days. Upon termination, all outstanding payments for services rendered up to the termination date shall be settled within fifteen (15) business days.
4. All patient records and medical data handled by the Ophthalmologist in China Guangzhou shall comply with the Personal Information Protection Law of the People's Republic of China and the Data Security Law.
5. Any disputes arising from this Quotation Estimate shall be resolved through arbitration under the China International Economic and Trade Arbitration Commission (CIETAC) in Guangzhou.
6. This Quotation Estimate is governed by the laws of the People's Republic of China, with specific reference to healthcare service regulations applicable in Guangdong Province and the city of Guangzhou.

The estimates provided in this Quotation Estimate assume standard operating conditions in China Guangzhou and do not account for extraordinary circumstances such as public health emergencies, government-mandated facility closures, or significant fluctuations in the CNY/USD exchange rate exceeding 5%. The Ophthalmologist's compensation is exclusive of any performance-based bonuses, which may be negotiated separately. All costs are stated in Chinese Yuan (CNY) unless otherwise indicated.
