3.1 Validity: This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any request for services beyond this period will require a revised Quotation Estimate reflecting current market rates in Nigeria Abuja.
3.2 Payment Terms: A non-refundable deposit of twenty-five percent (25%) of the total Quotation Estimate value is due upon acceptance. The remaining balance shall be payable in two equal instalments: fifty percent (50%) at the midpoint of the service delivery period and the final twenty-five percent (25%) upon completion of all ophthalmological services.
3.3 Professional Standards: The Ophthalmologist and all supporting clinical staff shall adhere strictly to the ethical guidelines of the Medical and Dental Council of Nigeria and the Ophthalmological Society of Nigeria. All patient records will be maintained in accordance with the National Health Act and data protection regulations applicable in Nigeria Abuja.
3.4 Cancellation and Rescheduling: In the event that the client wishes to cancel or reschedule any ophthalmological procedure outlined in this Quotation Estimate, a written notice of at least seventy-two (72) hours must be provided. Cancellations within seventy-two hours of a scheduled procedure will incur a fee of fifty percent (50%) of the booked session cost.
3.5 Liability and Insurance: Abuja Comprehensive Eye Care Centre maintains comprehensive professional indemnity insurance. The Ophthalmologist assumes full clinical responsibility for all procedures performed. In the unlikely event of a complication, the centre will provide all necessary corrective treatment at no additional cost to the client, as stipulated in this Quotation Estimate.
3.6 Taxation: All amounts stated in this Quotation Estimate are inclusive of Value Added Tax (VAT) at the prevailing rate of seven and a half percent (7.5%) as mandated by the Federal Inland Revenue Service of Nigeria. No additional levies or charges shall apply beyond those itemised above.
3.7 Dispute Resolution: Any disputes arising from the interpretation or execution of this Quotation Estimate shall be resolved through amicable negotiation. Should negotiation fail, the matter shall be referred to the Ophthalmological Society of Nigeria's dispute resolution panel in Nigeria Abuja for mediation.
