This Quotation Estimate is based on the clinical findings discussed during your initial consultation with our Optometrist in Australia Brisbane. The following assumptions apply to this estimate:
- All optical lens and frame selections are subject to final in-store confirmation. Substitution of materials, coatings, or frame styles may alter the final cost.
- The Medicare rebate of $86.85 is applied for patients under 65 years of age with no chronic medical conditions. Patients aged 65 and over, or those with a valid Health Care Card, may be eligible for a higher rebate of $154.70, which would reduce the total accordingly.
- Contact lens supply (ongoing monthly or quarterly lens purchases) is not included in this estimate and will be quoted separately upon fitting.
- Any additional diagnostic procedures identified during the examination (e.g., OCT scanning, visual field perimetry, or referral to an ophthalmologist) will be itemised as a supplementary quotation.
- All services are performed at our Fortitude Valley clinic in Brisbane, Queensland. Travel or home-visit fees do not apply to this estimate.
- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After this period, pricing may be subject to revision due to supplier cost fluctuations or changes in Australian healthcare rebate structures.
- Payment is due in full at the time of service delivery unless a written payment plan has been arranged in advance. We accept all major credit cards, EFTPOS, bank transfer, and HICAPS for eligible Medicare items.
- A 50% deposit is required to secure your selected optical frame and lens order. The balance is payable upon collection of your finished eyewear, typically within 7 to 10 business days.
- All work performed by our Optometrist team is covered under our professional indemnity insurance and complies with the Australian Health Practitioner Regulation Agency (AHPRA) standards of practice.
- Optical products carry a 12-month manufacturer's warranty against defects in materials and workmanship. This warranty does not cover damage caused by misuse, accidental breakage, or normal wear.
- This estimate does not constitute a binding contract. Final pricing is confirmed at the point of service. The practice reserves the right to amend this Quotation Estimate should clinical findings necessitate additional or alternative procedures.
- All personal health information collected during this process is handled in strict accordance with the Australian Privacy Act 1988 (Cth) and the National Privacy Principles.
