This Quotation Estimate has been prepared by our licensed Optometrist team at Alexandria Vision & Eye Care Centre to provide a comprehensive, itemised breakdown of the professional eye care services, diagnostic examinations, and corrective optical products recommended for the client listed above. Following the preliminary assessment, the attending Optometrist has noted a moderate myopic astigmatism in both eyes (OD: -2.75 / -1.25 × 180°; OS: -3.00 / -1.50 × 175°) with a mild presbyopic addition of +1.50 D. The corneal topography indicates a regular astigmatic pattern suitable for standard toric contact lens fitting. No pathological findings were observed in the retinal imaging or OCT scans. The Optometrist recommends the full diagnostic and corrective package outlined above to ensure optimal visual rehabilitation and long-term ocular health. All procedures will be performed in accordance with the standards set by the Egyptian Optometric Association and the Alexandria Health Directorate.
4.1 — A 50% advance payment (EGP 10,402.50) is required to confirm the booking and secure the Optometrist appointment slot. The remaining balance is due upon completion of all services and collection of optical products.
4.2 — Accepted payment methods include cash, bank transfer (CIB, NBE, or QNB Egypt), and major credit/debit cards. A 2% surcharge applies to card transactions.
4.3 — This Quotation Estimate is valid for 30 days. Prices are subject to revision based on fluctuations in the Egyptian Pound exchange rate or changes in supplier costs for imported optical materials.
4.4 — All spectacle lenses and frames carry a 12-month manufacturer warranty against defects in materials and workmanship. Contact lenses are non-returnable once the packaging is opened.
4.5 — Cancellation of a confirmed Optometrist appointment with less than 48 hours' notice will incur a 25% administrative fee on the total Quotation Estimate value.
Important Note: This Quotation Estimate is a good-faith projection and does not constitute a binding contract. Final charges may vary slightly if additional clinical findings necessitate supplementary Optometrist interventions not anticipated at the time of this estimate.
