Scope of Work and Service Delivery

This Quotation Estimate covers a full-cycle optometric programme to be executed by our senior Optometrist and a team of four (4) assistant optometrists at our primary clinic located in Osu, Ghana Accra, as well as at your designated premises within the Ghana Accra city limits. The programme is designed to accommodate up to fifty (50) patients over a period of three (3) consecutive working days. Each patient will receive a minimum of forty-five (45) minutes of dedicated Optometrist attention, ensuring thorough diagnostic evaluation and personalised corrective recommendations.

All diagnostic equipment used in this engagement is calibrated to international standards and maintained in accordance with the guidelines set by the Ghana Health Service and the Optometric Council of Ghana. The Optometrist leading this programme holds a Doctor of Optometry (OD) degree, is registered with the Optometric Council of Ghana, and possesses over twelve (12) years of clinical experience serving the diverse visual health population of Ghana Accra.

Terms and Conditions

1. Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision upon expiry due to fluctuations in the Ghanaian Cedi exchange rate and import costs for optical materials in Ghana Accra.

2. Payment Terms: A non-refundable deposit of fifty percent (50%) of the grand total is due upon acceptance of this Quotation Estimate. The remaining fifty percent (50%) shall be settled within fourteen (14) days of programme completion. Payment may be made via bank transfer to our account at Ecobank Ghana, Accra Branch, or by certified cheque.

3. Cancellation Policy: Cancellation by the client more than fourteen (14) days prior to the scheduled programme date will result in a 10% administrative fee. Cancellation within fourteen (14) days will forfeit the full deposit. The Optometrist practice reserves the right to reschedule in the event of force majeure.

4. Insurance and Liability: Our Optometrist practice carries professional indemnity insurance with a coverage limit of GHS 5,000,000. All services are performed in compliance with the Health Professions Regulatory Bodies Act, 2015 (Act 915) of Ghana.

5. Data Protection: All patient records and diagnostic images generated during this engagement shall be handled in strict accordance with the Data Protection Act, 2012 (Act 843) of Ghana. Records will be retained for a minimum of seven (7) years at our Ghana Accra facility.

6. Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved through mediation in Ghana Accra in accordance with the laws of the Republic of Ghana.
