Important Conditions Governing This Quotation Estimate
3.1 Validity: This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After this period, the Optometrist service rates in Iran Tehran may be subject to revision due to currency fluctuations, equipment costs, or regulatory changes.
3.2 Payment Terms: A 50% advance payment is required to schedule the Optometrist appointment. The remaining 50% balance is due upon completion of all services outlined in this Quotation Estimate. Payment may be made via bank transfer to the designated account of Teheran Vision & Eye Care Center, Tehran, Iran.
3.3 Scheduling: The Optometrist consultation sessions in Iran Tehran will be conducted at our clinic located on Vali-e-Asr Street. Appointments are available Monday through Saturday, 9:00 AM to 6:00 PM, excluding Iranian public holidays. The client must provide a minimum of 48 hours' notice for any rescheduling.
3.4 Cancellation Policy: Cancellations made less than 24 hours before a scheduled Optometrist session will incur a 25% fee of the session value as stated in this Quotation Estimate.
3.5 Scope Limitation: This Quotation Estimate covers Optometrist services only. It does not include ophthalmological surgical procedures, pharmaceutical prescriptions, or emergency eye care. Should the Optometrist identify a condition requiring referral to an ophthalmologist in Iran Tehran, a separate Quotation Estimate will be provided.
3.6 Confidentiality: All patient records, diagnostic images, and prescription data generated during the Optometrist services are protected under Iranian medical privacy regulations. No information will be disclosed to third parties without written consent from the client.
3.7 Acceptance: This Quotation Estimate becomes a binding agreement upon written acceptance and signature by both parties. The client's signature constitutes full acknowledgment of all Optometrist services, pricing, and terms described herein.
The Optometrist responsible for delivering the services in this Quotation Estimate holds a Doctor of Optometry (O.D.) degree from a recognized Iranian university and is registered with the Iranian Optometric Association. The clinic in Iran Tehran is equipped with digital phoropters, autorefractors, OCT scanners, corneal topographers, and A-scan biometers, ensuring that every Optometrist assessment meets international diagnostic accuracy standards. The facility is fully accessible and complies with all health and safety regulations set forth by the Tehran Municipality.
