This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Payment shall be made within fourteen (14) days of invoice issuance via bank transfer to our MCB Bank account (Account No. 0123-4567-8901, Branch: Westlands, Kenya Nairobi). A fifty percent (50%) advance payment is required to confirm the booking and mobilise our Optometrist team. The remaining balance shall be settled upon completion of all services and delivery of the final written report.
Terms and Conditions- All Optometrist services will be conducted in strict compliance with the Pharmacy and Poisons Control Board (PPCB) of Kenya and the Kenya Medical Practitioners and Dentists Council (KMPDC) guidelines.
- ClearSight Optometry Centre holds full professional indemnity insurance covering all Optometrist procedures performed in Kenya Nairobi.
- Spectacle frames and lenses are sourced from certified suppliers in Kenya Nairobi and come with a six-month manufacturer warranty against defects.
- Any additional services required beyond the scope outlined in this Quotation Estimate will be quoted separately and require written approval prior to execution.
- Confidentiality of all employee eye health records is guaranteed in accordance with the Data Protection Act, 2019 of Kenya.
- Scheduling of the on-site Optometrist examination will be coordinated with your HR department at least five (5) working days in advance.
- This Quotation Estimate does not constitute a binding contract until countersigned by both parties and the advance payment is received.
- Any disputes arising from this engagement shall be resolved through mediation in Kenya Nairobi in accordance with Kenyan law.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions outlined above for the Optometrist services to be delivered in Kenya Nairobi.
