- Validity: This Quotation Estimate is valid for thirty (30) days from the date of issue. Prices are subject to revision upon expiry without prior notice.
- Payment Terms: A 50% deposit is required upon acceptance of this Quotation Estimate. The remaining 50% balance is payable within fourteen (14) days of service completion. Payment shall be made via bank transfer to VisionCare Optometry Centre, Maybank Account No. 5123-4567-8901, Kuala Lumpur, Malaysia.
- Scheduling: The on-site Optometrist services will be conducted at the client's premises in Kuala Lumpur. A minimum of seven (7) working days' notice is required to confirm the service date.
- Warranty: All prescription spectacle lenses carry a six-month warranty against manufacturing defects. Frames carry a twelve-month warranty. Contact lens fittings include a free re-fit within three months if the initial prescription requires adjustment by the Optometrist.
- Confidentiality: All patient records, examination results, and prescription data collected during the Optometrist services shall be treated as strictly confidential in accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia.
- Cancellation: Cancellation of scheduled services more than five (5) working days in advance will incur no penalty. Cancellations within five (5) working days will be subject to a 25% cancellation fee on the total Quotation Estimate value.
- Licence & Compliance: VisionCare Optometry Centre holds a valid licence from the Malaysian Optical Dispensing Practitioners Board (MODPB) and operates in full compliance with all regulatory requirements for Optometrist practices in Malaysia Kuala Lumpur.
- Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved through mediation in Kuala Lumpur, Malaysia, in accordance with the laws of Malaysia.
