3.1 This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Beyond this period, the Optometrist reserves the right to revise pricing due to changes in material costs, import duties on optical equipment, or regulatory adjustments in Myanmar Yangon.
3.2 Payment is due within fourteen (14) days of acceptance of this Quotation Estimate. Accepted payment methods include bank transfer (KBZ Bank, CB Bank, or AYA Bank), cash payment at the clinic in Myanmar Yangon, and mobile payment via Wave Money or KBZPay.
3.3 A 50% advance payment is required to confirm the appointment and reserve the Optometrist's consultation slot. The remaining 50% balance is payable upon completion of all services and collection of the final prescription and written report.
3.4 All optometric services will be conducted at our primary clinic located in Kyauktada Township, Yangon, Myanmar. The Optometrist will perform all examinations personally and will not delegate diagnostic procedures to unlicensed personnel.
3.5 Cancellations must be made at least 48 hours in advance. Late cancellations or no-shows will incur a 25% fee of the total Quotation Estimate value to compensate for the reserved Optometrist consultation time.
3.6 This Quotation Estimate does not constitute a medical diagnosis or a guarantee of specific visual outcomes. The Optometrist will provide the most accurate assessment based on clinical findings, but individual patient responses to corrective lenses or contact lenses may vary.
3.7 All patient records, examination data, and prescription documents are the confidential property of the patient and are stored in compliance with Myanmar's data protection guidelines. The Optometrist will not disclose any information to third parties without written consent from the patient.
3.8 In the event of any dispute arising from the services described in this Quotation Estimate, both parties agree to first attempt resolution through amicable negotiation. If unresolved, the matter shall be referred to the Myanmar Optometric Association's dispute resolution panel in Yangon.
