This Quotation Estimate is issued by ClearVision Optometry Centre, a licensed and registered Optometrist practice located in the heart of Nepal Kathmandu, to provide a comprehensive and transparent breakdown of all professional vision care services, diagnostic procedures, and corrective optical products requested by the client. This document serves as a formal pricing proposal and does not constitute a binding contract until accepted in writing by both parties. All services described herein are delivered in accordance with the standards set by the Nepal Optometric Association and the Department of Health Services, Government of Nepal.
As a certified Optometrist operating in Nepal Kathmandu, our practice offers a full spectrum of eye care services tailored to the diverse visual needs of residents and visitors in the Kathmandu Valley. The following Quotation Estimate covers the complete package of services and products as discussed during the initial consultation on 10 June 2025.

- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiry date, prices may be revised due to fluctuations in import costs for optical products and changes in government tax policy in Nepal.
- All Optometrist services listed in this document are to be performed at our primary clinic located at Bhatbhateni Marg, Thamel, Nepal Kathmandu. Home-visit services are available at an additional charge of NPR 2,000 per visit within the Kathmandu Valley.
- Payment is due in full prior to the dispensing of prescription spectacles. We accept cash, bank transfer (Nabil Bank, NIC Asia, Himalayan Bank), and major credit/debit cards. A 50% advance payment is required to reserve lens manufacturing slots.
- Prescription lenses are custom-ordered and manufactured within 7 to 10 working days. Delays beyond this period due to supplier issues in Nepal will be communicated promptly, and no additional charges will apply.
- This Quotation Estimate does not include ophthalmological surgical procedures, emergency eye care, or hospitalization costs. In the event that the Optometrist identifies a condition requiring referral to an ophthalmologist or eye surgeon, a separate medical report and cost estimate will be provided.
- All diagnostic findings and prescriptions issued by our Optometrist in Nepal Kathmandu are confidential and protected under the Nepal Privacy Act, 2074 (2017). Records are retained for a minimum of five years.
- A warranty of six (6) months is provided on all prescription lenses against manufacturing defects. Frame warranty is one (1) year. Warranty claims must be presented with the original receipt and this Quotation Estimate document.
- Any cancellation of the order after lens manufacturing has commenced will incur a 40% non-refundable charge. Cancellation prior to manufacturing is fully refundable.
- This Quotation Estimate is subject to the laws of the Kingdom of Nepal. Any disputes arising from this document shall be resolved through the competent courts of Kathmandu, Nepal.
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions outlined above. The Optometrist practice at ClearVision Optometry Centre, Nepal Kathmandu, confirms that all services and products described herein will be delivered with the highest standard of professional care and ethical practice.
