1. Scope of Services

Each service will be delivered by a registered and certified Optometrist holding a valid licence from the Optometric Council of Nigeria (OCN) and the National Optometric Association of Nigeria (NOAN).

3. Terms and Conditions
  • This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision upon expiry due to fluctuations in the Nigeria Abuja market, particularly with respect to imported lens materials and optical equipment.
  • All Optometrist services will be performed in strict compliance with the standards set by the Optometric Council of Nigeria and the National Health Act, 2014. Practitioners will present valid professional licences upon request.
  • Payment terms: 40% advance payment upon acceptance of this Quotation Estimate, 40% upon completion of the on-site screening programme, and 20% upon final delivery of the annual optometric report. All payments shall be made in Nigerian Naira via bank transfer to the account details provided on the official invoice.
  • The on-site mobile screening unit will be deployed at the Federal Medical Centre, Abuja, premises. The client is responsible for providing a suitable consultation room (minimum 12 m²) with access to electricity and seating for patients.
  • Any additional services beyond the scope outlined in this Quotation Estimate will be subject to a separate written amendment and mutual agreement between both parties.
  • Abuja Vision Care & Optometry Centre shall maintain professional indemnity insurance with a minimum cover of NGN 50,000,000 for the duration of the engagement.
  • Confidentiality: All patient optometric records and data collected during the engagement shall be treated as strictly confidential in accordance with the Nigeria Data Protection Regulation (NDPR), 2019. Records will be retained for a minimum of seven (7) years.
  • Disputes arising from this Quotation Estimate or the subsequent service agreement shall be resolved through mediation in Abuja, FCT, Nigeria, in accordance with the Arbitration and Conciliation Act, Cap A18, LFN 2004.
4. Acceptance

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms and conditions stated herein. The engagement shall commence upon receipt of the 40% advance payment and execution of the formal Service Level Agreement (SLA).
