This Quotation Estimate is prepared by our senior Optometrist, Dr. Funke Adeyemi (NCOA Reg. No. NCOA/2019/00342), and covers all diagnostic and corrective services listed above. All examinations are conducted in our fully equipped clinic located in Victoria Island, Nigeria Lagos, using state-of-the-art diagnostic instruments calibrated to international standards. The Optometrist will provide a detailed written report following the initial consultation, which will be shared with the patient and, with written consent, with any referring ophthalmologist.
Please note that should the Optometrist identify a condition requiring referral to an ophthalmologist or further specialist investigation (e.g., OCT imaging, fluorescein angiography), additional costs will be communicated separately and are not included in this Quotation Estimate.
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision upon expiry due to fluctuations in the Nigeria Lagos foreign exchange market affecting imported optical materials.
- Payment is due within fourteen (14) days of acceptance. A 50% deposit is required to commence lens manufacturing and frame procurement. The balance is payable upon collection of the finished eyewear.
- All payments shall be made in Nigerian Naira (NGN) via bank transfer to the account details provided, or by POS at our Nigeria Lagos clinic. Cheques are not accepted.
- The Optometrist guarantees a 90-day workmanship warranty on all lens edging, frame adjustments, and dispensing. This does not cover damage caused by misuse, accidental breakage, or normal wear and tear.
- Lens and frame manufacturers' warranties (typically 12 months) apply separately and are transferable to the patient upon purchase.
- This Quotation Estimate is non-transferable and is prepared exclusively for the named client. Any changes to the scope of services will require a revised Quotation Estimate issued by the Optometrist.
- All patient data and examination records are handled in strict compliance with the Nigeria Data Protection Regulation (NDPR) 2019 and the professional code of ethics of the National Council for Optometry (NCOA).
- Delivery of finished spectacles is expected within seven (7) to ten (10) working days from the date of full payment, depending on lens availability in the Nigeria Lagos market.
- By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms stated herein. Acceptance does not constitute a binding contract until the deposit is received.
