As a registered Optometrist facility in Senegal Dakar, we adhere to the standards set forth by the Senegalese Ministry of Health and the National Order of Optometrists. Note regarding Senegal Dakar operations: All services listed in this Quotation Estimate are to be performed at our licensed Optometrist clinic located in the Plateau district of Senegal Dakar. In the event that the client requires home-visit optometric services within the greater Dakar area, an additional travel surcharge of 25,000 XOF per visit will apply. All optical products are sourced through certified international distributors and are guaranteed against manufacturing defects for a period of twelve (12) months from the date of dispensing. 4. Terms and Conditions of This Quotation Estimate- Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision based on exchange rate fluctuations affecting imported optical materials in Senegal Dakar.
- Payment Terms: A deposit of 50% is required upon acceptance of this Quotation Estimate to secure the Optometrist's appointment slot and to initiate the lens ordering process. The remaining 50% is due upon collection of the final optical products.
- Payment Methods: Payment may be made via bank transfer (Ecobank, BICICI, or Orange Money), cash, or certified cheque. All transactions are processed in CFA Francs (XOF) in accordance with Senegalese financial regulations.
- Warranty: All lenses and frames dispensed by our Optometrist practice carry a 12-month warranty against defects. Lens replacement due to incorrect prescription (verified by our Optometrist) is provided at no additional cost within 30 days of dispensing.
- Cancellation: In the event the client cancels after the 50% deposit has been paid, the deposit is non-refundable as it covers the Optometrist's reserved clinical time and pre-ordered materials.
- Regulatory Compliance: All optometric services are performed in full compliance with the Senegalese Public Health Code and the regulations of the National Order of Optometrists of Senegal. Our Optometrist holds a valid practicing license (No. OPT-SN-2018-0092) issued by the Ministry of Health of Senegal.
- Confidentiality: All patient records, prescription data, and visual health information are handled in strict accordance with Senegalese data protection laws. No information will be shared with third parties without written consent.
- Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. Failing resolution, matters shall be referred to the competent courts of Dakar, Senegal.
By signing below, the client acknowledges receipt of this Quotation Estimate for Optometrist services in Senegal Dakar and agrees to the terms and conditions outlined herein. The signed Quotation Estimate shall serve as a binding agreement between both parties for the services and products described.
