This Quotation Estimate is issued by ClearVision Optometry Centre, a licensed and registered Optometrist practice operating in Singapore Singapore, to provide Mr. Tan Wei Ming with a comprehensive and transparent breakdown of all anticipated costs associated with the recommended optometric services. This document serves as a formal Quotation Estimate and does not constitute a binding contract until accepted in writing by the client. Each service below has been carefully selected by our Optometrist team based on clinical assessment and is itemised in this Quotation Estimate for the client's review and approval.

The total amount stated in this Quotation Estimate shall be payable as follows: a 50% deposit (SGD 722.13) is due upon acceptance of this Quotation Estimate to reserve the Optometrist appointment slot and commence lens manufacturing. The remaining 50% balance (SGD 722.12) is payable upon collection of the completed spectacles or contact lenses at our clinic in Singapore Singapore. Accepted payment methods include cash, NETS, credit/debit card (Visa, Mastercard, AMEX), PayNow, and bank transfer. No interest or late fees will be charged on the balance if settled within 14 days of the collection date.
Standard prescription spectacles will be ready for collection within 5 to 7 working days from the date of deposit payment. Contact lens fitting and trial will be scheduled within 3 working days of the initial Optometrist examination. All services will be delivered at our premises located at 128 Orchard Road, Singapore Singapore 238849. The client will be notified via SMS or email once the order is ready for collection.
- This Quotation Estimate is valid for 30 calendar days from the date of issue. After the expiry date, prices may be revised due to changes in lens supplier costs, exchange rates, or regulatory adjustments in Singapore Singapore.
- All Optometrist services are performed by registered and licensed optometrists under the Ministry of Health (MOH) Singapore. The client's personal health data will be handled in strict accordance with the Personal Data Protection Act (PDPA) of Singapore.
- Custom-made lenses are non-refundable once the manufacturing process has commenced. However, a free re-fitting or minor adjustment is available within 30 days of collection if the client experiences discomfort or visual dissatisfaction, subject to our Optometrist's clinical assessment.
- This Quotation Estimate does not include the cost of any additional diagnostic procedures, emergency treatments, or referrals to ophthalmologists that may be recommended during or after the examination.
- The client acknowledges that this Quotation Estimate is an estimate of costs and that the final invoice may vary by up to 5% depending on the specific lens parameters determined during the detailed Optometrist fitting session.
- All services are subject to the availability of the specific Optometrist and equipment at our Singapore Singapore clinic. Rescheduling may be required in the event of unforeseen circumstances, and the client will be given at least 48 hours' notice.
- By signing below, the client confirms acceptance of this Quotation Estimate and authorises ClearVision Optometry Centre to proceed with the outlined Optometrist services in Singapore Singapore.
