Important Conditions Governing This Quotation Estimate
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, prices may be revised to reflect changes in the Tanzania Dar es Salaam market for optical materials and professional Optometrist fees.
- Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, and the remaining 50% within fourteen (14) days of completion of all services. Payments shall be made via bank transfer to Dar Vision Optometry Centre, NMB Bank, Dar es Salaam Branch, Account No. 0012-3456-7890.
- All Optometrist services shall be performed by licensed and registered Optometrist professionals holding valid practising certificates from the Tanzania Medical Council. The lead Optometrist for this engagement shall be Mr. Emmanuel Kessy, BSc Optometry, MSc Clinical Optometry.
- The client agrees to provide a suitable, well-ventilated space for the on-site mobile Optometry Unit operations within the school premises in Dar es Salaam.
- Any additional services beyond the scope defined in this Quotation Estimate shall be subject to a separate written amendment and additional charges at the standard Optometrist consultation rate of TZS 50,000 per session.
- All patient data collected during the Optometrist examination shall be handled in strict accordance with the Tanzania Data Protection Act, 2022, and shall be stored securely for a minimum of five (5) years.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and countersigned by authorised representatives.
- Disputes arising from this Quotation Estimate shall be resolved through amicable negotiation, failing which they shall be referred to the Tanzania Arbitration Centre in Dar es Salaam.
