1. Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Prices are subject to revision after the expiry date due to fluctuating material costs in the United Arab Emirates Dubai market.
2. Payment Terms: A 50% advance payment is required to confirm the booking. The remaining 50% balance is due upon completion of all Optometrist services and delivery of corrective eyewear. Payment may be made via bank transfer, corporate cheque, or approved credit card. Bank details: Emirates NBD, Account Name: ClearVision Optometry Center FZ-LLC, IBAN: AE07 0240 0000 0000 1234 56.
3. Scheduling: All Optometrist appointments will be coordinated with the client's HR department. A minimum of five (5) business days' notice is required to reschedule any session. Sessions are conducted Monday through Saturday, 9:00 AM to 7:00 PM, at our United Arab Emirates Dubai facility.
4. Warranty: All prescription lenses carry a twelve (12)-month manufacturing defect warranty. Frames carry a six (6)-month structural warranty. Warranty claims must be submitted in writing to our Optometrist team within the warranty period.
5. Cancellation Policy: Cancellations made more than 72 hours before the scheduled appointment incur no charge. Cancellations within 72 hours are subject to a 25% fee of the booked service value.
6. Regulatory Compliance: All Optometrist services are performed in accordance with the United Arab Emirates Federal Law No. 3 of 2004 on Health Care Practice and its amendments, as well as Dubai Health Authority (DHA) operational guidelines. This Quotation Estimate does not constitute a medical diagnosis; all findings will be communicated in a separate clinical report.
7. Data Protection: Client and employee health data will be handled in strict compliance with the United Arab Emirates Federal Decree-Law No. 45 of 2021 on the Protection of Personal Data. No information will be shared with third parties without written consent.
8. Dispute Resolution: Any disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. Failing resolution, matters shall be referred to the Dubai Courts in accordance with the laws of the United Arab Emirates Dubai jurisdiction.
