3.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, the Optometrist service fees and associated costs may be subject to revision based on prevailing rates in Uzbekistan Tashkent.
3.2 Payment Terms: Payment is due within fourteen (14) days of acceptance of this Quotation Estimate. Accepted payment methods include bank transfer to the account of Tashkent Vision Care Center (IBAN: UZ00 9850 0000 0000 0000 0000), cash at the Tashkent facility, or international wire transfer in USD. A 50% advance payment is required to schedule the Optometrist appointment.
3.3 Cancellation Policy: Should the client wish to cancel any scheduled Optometrist session, a written notice of at least 48 hours prior to the appointment is required. Failure to provide such notice will result in a 25% cancellation fee on the applicable service line item.
3.4 Scope Limitations: This Quotation Estimate covers only the Optometrist services explicitly listed in Section 2. Any additional diagnostic tests, emergency eye care, or ophthalmological surgical referrals that may be identified during the examination are not included and will be quoted separately.
3.5 Regulatory Compliance: All Optometrist services are delivered in full compliance with the regulations of the Ministry of Health of the Republic of Uzbekistan and the standards set by the Uzbekistan Optometric Association. The facility in Tashkent operates under Licence No. UZ-MH-2020-1187.
3.6 Confidentiality: All patient records, refraction data, and diagnostic images obtained during the Optometrist examination are protected under the data privacy laws of Uzbekistan. No information will be shared with third parties without the explicit written consent of the client.
3.7 Governing Law: This Quotation Estimate and any resulting service agreement shall be governed by and interpreted in accordance with the civil and commercial laws of the Republic of Uzbekistan. Any disputes shall be resolved through the competent courts of Tashkent, Uzbekistan.
