Terms and Conditions of this Quotation Estimate
- This Quotation Estimate is valid for a period of thirty (30) days from the date of issue. After this period, prices may be subject to revision due to fluctuations in the Zimbabwe Harare market, particularly regarding imported lens materials and frame inventory.
- Payment terms: Fifty percent (50%) deposit is required upon acceptance of this Quotation Estimate to secure the booking of the Optometrist's schedule. The remaining fifty percent (50%) is due within fourteen (14) days of service completion.
- All optometric examinations and lens dispensing will be conducted by a fully registered and licensed Optometrist in compliance with the Health Professions Act of Zimbabwe and the regulations of the Zimbabwe Optometrists Association.
- The annual vision screening programme (Item 7) includes one (1) comprehensive examination per employee per calendar year, conducted at our clinic in Zimbabwe Harare or via the mobile unit as scheduled.
- On-site mobile visits (Item 8) are limited to the Harare metropolitan area. Travel to locations outside Harare (e.g., Bulawayo, Mutare, Gweru) will incur additional transport and accommodation charges to be agreed upon in writing.
- This Quotation Estimate does not include surgical referrals, ophthalmological consultations, or treatment of ocular diseases. Should the Optometrist identify a condition requiring referral to an ophthalmologist, the client will be advised accordingly, and such services are outside the scope of this estimate.
- All spectacle frames and lenses carry a manufacturer's warranty of twelve (12) months against defects in materials and workmanship. Damage due to misuse, accidental breakage, or improper storage is not covered under warranty.
- Confidentiality: All patient records and examination data collected during the provision of services will be handled in strict accordance with Zimbabwe's data protection regulations and the professional ethical code of the Optometrist.
- Disputes arising from this Quotation Estimate shall be resolved through amicable negotiation. In the event of unresolved disputes, the matter shall be referred to the Small Claims Court in Harare, Zimbabwe, in accordance with the jurisdiction of Zimbabwe Harare.
- This Quotation Estimate is non-transferable and is prepared exclusively for the client named above. Any modification to the scope of services requires a revised Quotation Estimate issued in writing by the Optometrist.
