1. This Quotation Estimate is valid for thirty (30) days from the date of issue. Any changes in scope, duration, or service location within Malaysia Kuala Lumpur may result in a revised Quotation Estimate.
2. All Paramedic personnel deployed under this agreement shall hold valid Malaysian Paramedic Council registration, current MOH medical fitness certificates, and relevant insurance coverage.
3. Payment terms: 50% advance payment upon acceptance of this Quotation Estimate; the remaining 50% shall be settled within fourteen (14) days from the end of each service month.
4. The client shall provide a designated safe area for Paramedic station setup, access to electrical power, and adequate parking for ambulance vehicles at the service location in Malaysia Kuala Lumpur.
5. Cancellation policy: Cancellation with less than 72 hours' notice for scheduled Paramedic deployments shall incur a 50% fee of the booked service value.
6. All medical records and patient information handled by our Paramedic team shall be treated in strict confidence in accordance with the Personal Data Protection Act 2010 (PDPA) of Malaysia.
7. This Quotation Estimate does not include costs for hospital admission, specialist consultations, or medical supplies consumed beyond the standard Paramedic emergency kit.
8. The service contract period is twelve (12) months, renewable upon mutual written agreement. Early termination requires sixty (60) days' written notice.
