This Quotation Estimate is issued by MediRescue Manila Paramedic Services Inc. to provide comprehensive, round-the-clock Paramedic emergency medical response services within the Philippines Manila metropolitan area and its immediate surrounding provinces. The scope encompasses advanced life support (ALS) ambulance dispatch, on-site Paramedic intervention, patient stabilization, inter-facility transfers, and post-incident medical documentation. All Paramedic personnel deployed under this arrangement are licensed by the Philippine Board of Nursing and hold current Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications recognized by the Department of Health of the Philippines.

- This Quotation Estimate is valid for thirty (30) calendar days from the date of issuance. Beyond this period, pricing may be subject to revision based on fuel surcharges, minimum wage adjustments in the Philippines Manila region, or changes in Philippine government regulations governing emergency medical services.
- Payment terms: Net 30 days from the date of invoice. A 1.5% monthly interest shall apply to overdue balances. Payment shall be made via bank transfer to the account of MediRescue Manila Paramedic Services Inc. at BDO Unibank, Makati Branch.
- All Paramedic personnel assigned under this contract shall comply with the Philippine Nursing Act of 2002 (RA 9173), the DOH Administrative Order No. 2019-004 on Emergency Medical Services, and all applicable Metro Manila development regulations.
- MediRescue Manila Paramedic Services Inc. guarantees a maximum response time of eight (8) minutes from dispatch to on-scene arrival within the Philippines Manila core service area (Quezon City, Makati, Pasig, Taguig, Mandaluyong, and Marikina).
- The client reserves the right to audit service records, Paramedic credentials, and vehicle inspection logs upon thirty (30) days written notice.
- Either party may terminate this agreement with sixty (60) days written notice. In the event of termination, a pro-rated refund of prepaid services shall be issued within fifteen (15) business days.
- This Quotation Estimate does not constitute a binding contract until a formal Service Agreement is executed by both parties and countersigned by their respective legal counsel.
- All personal data collected during Paramedic interventions shall be handled in strict compliance with the Philippine Data Privacy Act of 2012 (RA 10173).
By signing below, the client acknowledges receipt of this Quotation Estimate for Paramedic services in the Philippines Manila area and agrees to the terms and conditions stated herein. This acceptance does not obligate either party to enter into a formal contract until all due diligence and procurement requirements have been satisfied.
