This Quotation Estimate outlines the professional services to be provided by a licensed Pharmacist operating within the jurisdiction of Canada Vancouver. The Pharmacist will be engaged on a full-time, twelve-month contract basis to deliver comprehensive pharmaceutical care, clinical consultation, medication management, and regulatory compliance services across all Coastal Health Clinics Group locations in the Vancouver metropolitan area. The Pharmacist shall hold a valid license issued by the College of Pharmacists of British Columbia and shall comply with all provincial and federal regulations governing the practice of pharmacy in Canada Vancouver.

The scope of this Quotation Estimate encompasses the following core responsibilities for the Pharmacist:

  • Dispensing and verification of all prescribed medications in accordance with British Columbia pharmacy regulations.
  • Conducting clinical drug reviews, therapeutic drug monitoring, and patient counselling sessions.
  • Managing the pharmacy inventory, including procurement, storage, and disposal of controlled substances per Health Canada guidelines.
  • Providing immunization services, including influenza, pneumococcal, and travel vaccinations, as authorized under the College of Pharmacists of BC scope of practice.
  • Collaborating with physicians, nurses, and allied health professionals within the Canada Vancouver healthcare network to optimize patient medication regimens.
  • Maintaining accurate patient medication records and ensuring compliance with PHIPA (Personal Health Information Protection Act) and all applicable privacy legislation in Canada Vancouver.

  • This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, pricing for the Pharmacist engagement and associated services in Canada Vancouver may be subject to revision based on market conditions and regulatory changes.
  • All amounts stated in this Quotation Estimate are in Canadian Dollars (CAD) and are inclusive of the applicable Goods and Services Tax (GST) and Provincial Sales Tax (PST) as levied in the province of British Columbia, Canada Vancouver.
  • The Pharmacist named or assigned under this Quotation Estimate must maintain an active, unrestricted license with the College of Pharmacists of British Columbia for the entire duration of the contract. Any lapse in licensure will constitute a material breach of this agreement.
  • Payment terms: 50% of the total Quotation Estimate amount is due upon contract execution, and the remaining 50% shall be paid in equal monthly installments over the twelve-month engagement period. Late payments will incur a 1.5% monthly interest charge.
  • The client reserves the right to request a replacement Pharmacist if the originally assigned professional is unable to fulfill duties due to illness, resignation, or disciplinary action. Any replacement Pharmacist must meet the same licensure and competency standards required in Canada Vancouver.
  • All pharmaceutical records, patient data, and clinical documentation generated under this Quotation Estimate shall remain the property of the client and must be handled in strict accordance with PHIPA and the federal Privacy Act of Canada.
  • This Quotation Estimate does not constitute a binding contract until countersigned by both parties. A formal Service Agreement will be executed upon acceptance of this Quotation Estimate.
  • Any changes to the scope of services, additional locations in the Vancouver area, or extended hours for the Pharmacist will require a written amendment to this Quotation Estimate and may result in adjusted pricing.

By signing below, the client acknowledges receipt of this Quotation Estimate for Pharmacist services in Canada Vancouver and agrees to the terms, conditions, and pricing outlined herein. This Quotation Estimate represents a good-faith estimate of costs and is subject to the final Service Agreement to be executed by both parties.
