1. Scope of Quotation Estimate

This Quotation Estimate is issued by MediPharm Solutions Indonesia to provide a comprehensive and transparent cost breakdown for the recruitment, deployment, and ongoing professional services of a licensed Pharmacist within the Indonesia Jakarta metropolitan area. The scope encompasses all administrative, regulatory, clinical, and operational requirements necessary to ensure that the Pharmacist operates in full compliance with the Indonesian Ministry of Health (Kementerian Kesehatan RI) regulations, the Indonesian Pharmacists Association (Ikatan Apoteker Indonesia – IAI), and all applicable local ordinances governing pharmaceutical practice in DKI Jakarta.

The Pharmacist services outlined in this Quotation Estimate are designed to support PT Sehat Nusantara Healthcare Group's outpatient pharmacy operations, inpatient medication management, clinical drug information services, and patient counselling programmes across their three (3) facility locations in Indonesia Jakarta. All services will be delivered by a Pharmacist holding a valid Surat Izin Praktik Apoteker (SIPA) and a current registration with the IAI, ensuring the highest standard of pharmaceutical care in the Indonesia Jakarta region.

3. Terms and Conditions of This Quotation Estimate

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Any acceptance beyond this period may be subject to revised pricing due to fluctuations in the Indonesia Jakarta pharmaceutical labour market.
All prices stated in this Quotation Estimate are denominated in Indonesian Rupiah (IDR) and are inclusive of all applicable local levies in DKI Jakarta unless otherwise noted. The 11% Value Added Tax (Pajak Pertambahan Nilai – PPN) is itemised separately.
The Pharmacist deployed under this agreement shall be bound by a minimum twelve (12) month service contract. Early termination by either party prior to the completion of the contract term shall incur a penalty equivalent to three (3) months of the Pharmacist's professional fee.
MediPharm Solutions Indonesia guarantees that the Pharmacist will hold a valid and current SIPA, IAI membership, and all necessary certifications required by the Indonesia Jakarta health authority (Dinas Kesehatan DKI Jakarta) for the entire duration of the engagement.
Payment terms: 30% advance payment upon acceptance of this Quotation Estimate, 40% upon successful onboarding and first month of service delivery, and 30% upon completion of the twelfth month. All payments shall be made via bank transfer to the account specified in the attached invoice.
Any changes to the scope of Pharmacist services, additional facility locations within Indonesia Jakarta, or modifications to the clinical service model must be documented in a formal Change Order signed by both parties before implementation.
This Quotation Estimate does not constitute a binding contract. A separate Service Level Agreement (SLA) and Master Service Agreement (MSA) will be executed upon mutual acceptance of the terms herein.
All personal data of the Pharmacist and patient information handled under this engagement shall be processed in strict accordance with Indonesia's Personal Data Protection Law (UU No. 27 Tahun 2022) and the healthcare confidentiality standards of the Indonesia Jakarta medical community.
Disputes arising from this Quotation Estimate or the subsequent service agreement shall be resolved through mediation in Indonesia Jakarta, and if unresolved, through the competent courts of the Jakarta Central District Court (Pengadilan Negeri Jakarta Pusat).

4. Acceptance and Authorisation

By signing below, the client acknowledges receipt of this Quotation Estimate for Pharmacist services in Indonesia Jakarta and agrees to the terms and conditions outlined herein. This document serves as the formal basis for the subsequent execution of the Master Service Agreement between MediPharm Solutions Indonesia and PT Sehat Nusantara Healthcare Group.
