- This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiry date, the Pharmacist reserves the right to revise the rates and terms presented herein.
- All services described in this Quotation Estimate shall be performed by a Pharmacist duly registered with the Dutch Council for Health Care Professions (BIG-register) and holding a valid licence to practise in Amsterdam, Netherlands.
- Payment terms: 50% of the total Quotation Estimate amount is due upon acceptance of this document; the remaining 50% is payable within thirty (30) days of completion of all services. Invoices shall be issued in accordance with Dutch tax regulations (BTW).
- The Pharmacist shall comply with all applicable data protection requirements under the Dutch Implementation Act of the GDPR (UAVG) and the professional confidentiality obligations set forth in the Dutch Code of Conduct for Pharmacists.
- Any additional services beyond the scope defined in this Quotation Estimate shall be subject to a separate written amendment and additional Quotation Estimate prior to commencement.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties. Acceptance may be communicated via email or signed copy of this document.
- All disputes arising from this Quotation Estimate shall be governed by the laws of the Netherlands and shall be subject to the exclusive jurisdiction of the competent court in Amsterdam, Netherlands.
- The Pharmacist shall carry professional liability insurance with a minimum coverage of €1,000,000 per claim, as required for pharmaceutical practice in the Netherlands.
