Terms and Conditions of this Quotation Estimate
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, pricing may be adjusted to reflect changes in the Peru Lima pharmaceutical labor market or regulatory fee structures.
- All Pharmacist services described herein shall be performed by professionals holding a valid license issued by the Peruvian Ministry of Health (MINSA) and registered with the Colegio de Farmacéuticos del Perú. The lead Pharmacist for this engagement is Lic. María Elena Quispe Rojas (CIP: 123456).
- Payment terms: 50% advance upon acceptance of this Quotation Estimate, and 50% upon completion of the contracted service period. Payments shall be made via bank transfer to the account of Farmacia Integral del Pacífico S.A.C., Banco de Crédito del Perú, Cta. Cte. 194-00234567-0-12, Lima, Peru.
- All services will be rendered at the client's facility in Peru Lima or at our registered office in San Isidro, Lima, as mutually agreed. Travel within the Lima metropolitan area is included; travel to other regions of Peru will be quoted separately.
- The Pharmacist team shall comply with all applicable Peruvian regulations, including D.S. N° 007-2002-SA (Pharmaceutical Practice), DIGEMID guidelines, and the General Law of Health (Ley General de Salud, D.L. 790).
- Confidentiality: All patient data, formulary information, and institutional records accessed during the provision of Pharmacist services in Peru Lima shall be treated as strictly confidential in accordance with Peruvian data protection law (Ley N° 29733).
- Any modifications to the scope of work outlined in this Quotation Estimate must be approved in writing by both parties prior to execution. Additional services will be subject to a supplementary Quotation Estimate.
- This Quotation Estimate does not constitute a binding contract until formally accepted in writing by the client and countersigned by the authorized representative of Farmacia Integral del Pacífico S.A.C.
