The following Quotation Estimate outlines all costs associated with the Pharmacist engagement in Thailand Bangkok. All figures are expressed in Thai Baht (THB) and are inclusive of applicable Value Added Tax (VAT) at 7% as mandated by the Revenue Department of Thailand.

Payment for this Quotation Estimate shall be made via bank transfer to Siam Wellness Healthcare Group Co., Ltd. at Siam Commercial Bank, Sukhumvit Branch, Thailand Bangkok. The payment schedule is as follows:

• 30% advance payment (THB 508,303.50) due upon signed acceptance of this Quotation Estimate, prior to commencement of Pharmacist recruitment.
• Monthly instalments of THB 113,037.08 for the Pharmacist professional fee, CPD, and administrative fee, payable by the 5th business day of each month.
• Remaining balance of one-time fees (recruitment, verification, onboarding, insurance) to be settled within 14 days of service completion.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all rates are subject to revision based on prevailing market conditions in Thailand Bangkok.
• The Pharmacist engaged under this Quotation Estimate must maintain an active license with the Thai Pharmacy Council throughout the contract period. Any lapse in licensure shall result in immediate suspension of services and a replacement Pharmacist shall be sourced at no additional recruitment cost.
• All services shall be performed in strict compliance with the Thai Pharmaceutical Act B.E. 2510 (1967), the Thai FDA regulations, and the Hospital Licensing Act applicable to facilities in Thailand Bangkok.

• Any disputes arising from this Quotation Estimate shall be resolved through the Bangkok Arbitration Institute in accordance with its rules of procedure.
