2. Scope of Services – Physiotherapist Professional
This Quotation Estimate is issued by Rio Physio & Rehabilitation Center to provide comprehensive Physiotherapist professional services for the benefit of the client's corporate employees, to be delivered at our facility located in Brazil, Rio de Janeiro. The services encompass a full range of physiotherapy interventions designed to promote recovery, prevent workplace injuries, and enhance the overall musculoskeletal health of the client's workforce.
The Physiotherapist team assigned to this contract consists of four (4) licensed professionals, each holding a valid registration with CREFITO-2 and a minimum of five (5) years of clinical experience in orthopedic, neurological, and sports physiotherapy. All practitioners operate in full compliance with the regulations established by the Conselho Federal de Fisioterapia e Terapia Ocupacional (CFFITO) and the applicable municipal health authority of Rio de Janeiro.

4. Terms and Conditions
- Validity: This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After this period, prices may be subject to revision based on market conditions in Brazil, Rio de Janeiro.
- Payment Terms: Payment shall be made via bank transfer (TED/PIX) to the account details provided by Rio Physio & Rehabilitation Center. Invoices will be issued monthly, due within fifteen (15) business days of receipt. A late payment penalty of 1% per month plus SELIC interest will apply as per Brazilian commercial law (Código Civil, Art. 412).
- Contract Duration: The Physiotherapist services described herein are offered under a twelve (12) month contract, renewable by mutual written agreement of both parties.
- Confidentiality: All patient records, clinical data, and corporate information shall be treated in strict accordance with the Brazilian General Data Protection Law (LGPD – Lei Geral de Proteção de Dados, Lei nº 13.709/2018). The Physiotherapist team is bound by professional secrecy as mandated by the CFFITO Code of Ethics.
- Cancellation Policy: Either party may terminate the contract with a thirty (30) day written notice. In the event of early termination by the client, a cancellation fee equivalent to 10% of the remaining contract value will be applied.
- Liability: Rio Physio & Rehabilitation Center maintains professional liability insurance (Seguro de Responsabilidade Profissional) with a coverage of R$ 2,000,000.00, valid throughout the state of Rio de Janeiro, Brazil.
- Governing Law: This Quotation Estimate and any resulting contract shall be governed by the laws of the Federative Republic of Brazil. Any disputes shall be resolved by the competent courts of the city of Rio de Janeiro, RJ.
- Service Location: All Physiotherapist consultations and treatments will be conducted at our facility in Copacabana, Rio de Janeiro, or at the client's premises in the city of Rio de Janeiro, as mutually agreed upon in the service schedule.
5. Acceptance and Authorization
By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms, conditions, and pricing outlined in this document for the provision of Physiotherapist services in Brazil, Rio de Janeiro.
