The Physiotherapist assigned to this contract holds a professional degree in Kinesioterapia (Physiotherapy) from the Universidad de Chile, is registered with the Colegio de Kinesiólogos de Chile (CCK) under license number CCK-2019-4471, and possesses a minimum of eight years of clinical experience specializing in occupational musculoskeletal rehabilitation. The practitioner is additionally certified in the FMS (Functional Movement Screen) methodology and holds a valid first-aid and CPR certification issued by the Cruz Roja Chilena. All services will be delivered in compliance with the regulations established by the Superintendencia de Seguridad Social (SUSESO) and the applicable norms of the Superintendencia del Medio Ambiente regarding workplace health in Chile Santiago.
- Validity: This Quotation Estimate remains valid for thirty (30) calendar days from the date of issue. Beyond this period, pricing may be adjusted to reflect current market conditions in Chile Santiago.
- Payment Terms: Invoices will be issued on the first business day of each month for services rendered in the preceding month. Payment is due within fifteen (15) calendar days via transferencia bancaria to the account specified on the invoice. A late-payment surcharge of 1.5% per month will apply.
- Service Period: The engagement of the Physiotherapist is proposed for an initial term of twelve (12) months, renewable by mutual written agreement for successive periods of six (6) months.
- Cancellation: Either party may terminate the agreement with thirty (30) days prior written notice. In the event of early termination by the client, a cancellation fee equivalent to one month's service fee will be applicable.
- Confidentiality: All patient records, functional assessments, and progress reports generated by the Physiotherapist are subject to the Ley 19.628 on the Protection of Private Life and the data protection regulations of Chile. Information will not be disclosed to third parties without written consent.
- Liability & Insurance: Clínica de Rehabilitación Andina SpA maintains a professional liability insurance policy (póliza de responsabilidad civil profesional) with a coverage limit of 500.000.000 CLP, issued by Compañía de Seguros Andina, valid throughout the metropolitan region of Chile Santiago.
- Dispute Resolution: Any dispute arising from this Quotation Estimate or the resulting contract shall be resolved through mediation before the Cámara de Comercio de Santiago. If mediation fails, the parties submit to the ordinary courts of Santiago, Chile.
- Force Majeure: Neither party shall be liable for delays or non-performance caused by events beyond reasonable control, including but not limited to natural disasters, government-mandated closures, or public health emergencies as declared by the Ministry of Health of Chile.
By signing below, the client acknowledges that this Quotation Estimate accurately reflects the scope, pricing, and terms for the Physiotherapist services to be provided in Chile Santiago. Upon countersignature by both parties, this document shall constitute a binding service agreement.
