The Physiotherapist assigned to this case, Ms. Nour El-Din (B.Sc. Physiotherapy, M.Sc. Sports Rehabilitation, licensed by the Egyptian Physiotherapy Association), will oversee all sessions and coordinate with the client's surgical team as needed. The program spans twelve weeks and includes a combination of manual therapy, therapeutic exercise, electrotherapy, and patient education.

The total amount of this Quotation Estimate shall be payable in three installments: 40% upon acceptance of this document, 30% at the midpoint of the twelve-week program (end of Week 6), and the remaining 30% upon completion of the final session. Payment may be made via bank transfer to Alexandria Coastal Physiotherapy & Rehabilitation Center (Bank: National Bank of Egypt, Branch: Smouha, Alexandria), or by credit/debit card at our front desk. A receipt will be issued for every transaction in compliance with Egyptian financial regulations.

4.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, pricing may be subject to revision based on current market rates in Egypt Alexandria.

4.2 Scheduling: The Physiotherapist will coordinate session times with the client. Sessions are available Monday through Saturday, 8:00 AM to 8:00 PM. Rescheduling must be communicated at least 24 hours in advance; otherwise, a 50% session fee will apply.

4.3 Cancellation: Should the client need to cancel the program before Week 4, a 20% administrative fee will be deducted from any prepaid amount. Cancellations after Week 4 will be non-refundable for sessions already completed.

4.4 Medical Clearance: The client must provide a current medical clearance from the referring physician before commencing any Physiotherapist-led intervention. Our team in Egypt Alexandria will not initiate treatment without verified medical authorization.

4.5 Confidentiality: All patient records, treatment notes, and progress reports are handled in strict accordance with Egyptian data protection laws and the ethical code of the Egyptian Physiotherapy Association.

4.6 Liability: The Physiotherapist and the clinic assume no liability for pre-existing conditions not disclosed at the initial assessment. The client agrees to report any adverse symptoms immediately during or after sessions.

4.7 Governing Law: This Quotation Estimate and any resulting service agreement shall be governed by the laws of the Arab Republic of Egypt. Any disputes shall be resolved through the competent courts in Alexandria, Egypt.

By signing below, the client acknowledges receipt of this Quotation Estimate for Physiotherapist services in Egypt Alexandria, agrees to the terms and conditions outlined herein, and authorizes the commencement of the described treatment program upon payment of the initial installment.
