The Physiotherapist programme outlined in this Quotation Estimate is structured over a period of twelve (12) weeks, commencing on 1 July 2025 and concluding on 28 September 2025. All sessions will be conducted at the Asante Physiotherapy & Rehabilitation Centre, 14 Independence Avenue, Osu, Ghana Accra, unless otherwise agreed in writing. The Physiotherapist will maintain a detailed clinical record for each session and will provide a written progress summary to the client at the end of every four-week cycle. Should the Physiotherapist determine that additional sessions are required beyond the twelve-week period, a supplementary Quotation Estimate will be issued for the client's approval before any further treatment commences.
Payment for the services detailed in this Quotation Estimate shall be made in three equal instalments: (i) 40% upon signing and acceptance of this document, (ii) 30% at the midpoint of the twelve-week programme (end of Week 6), and (iii) the remaining 30% upon completion of the final session. All payments are to be made in Ghana Cedis (GHS) via bank transfer to the account details provided on the invoice. A 2% late-payment surcharge will apply to any instalment not settled within fourteen (14) calendar days of the due date. This Quotation Estimate is valid for thirty (30) days from the date of issue; beyond this period, the Physiotherapist reserves the right to revise the pricing due to potential changes in consumable costs, utility charges, or regulatory adjustments by the Physiotherapy Council of Ghana.
The client is requested to provide a minimum of twenty-four (24) hours' notice for any rescheduling or cancellation of a Physiotherapist session. Cancellations made within twenty-four hours of the scheduled appointment will be subject to a 50% fee of the session cost. In the event that the Physiotherapist is unable to attend a scheduled session due to illness or emergency, the session will be rescheduled at no additional cost to the client, and no fee will be charged for the missed appointment.
Important Note: This Quotation Estimate is prepared exclusively for the named client and is not transferable. All Physiotherapist services are subject to the clinical judgement of the treating practitioner. The Physiotherapist may modify the treatment plan at any point if the client's condition warrants a change in approach, and such modifications will be communicated in writing.
