This Quotation Estimate is formally issued by Al-Shifa Physiotherapy & Rehabilitation Center to provide Al-Rashid Industrial Group with a comprehensive, itemized financial proposal for the engagement of a qualified Physiotherapist to deliver on-site rehabilitation and preventive care services at their Jeddah facility. This document outlines the full scope of services, associated costs, payment terms, and conditions governing the professional relationship between both parties within the jurisdiction of Saudi Arabia Jeddah.
The contracted Physiotherapist shall be a licensed professional registered with the Saudi Commission for Health Specialties (SCFHS) and shall provide the following services at the client's premises in Jeddah:
- Preventive ergonomic assessments for all industrial workers (minimum 200 employees)
- Individualized rehabilitation programs for musculoskeletal injuries
- Manual therapy, therapeutic exercise, and electrotherapy modalities
- Post-surgical rehabilitation and return-to-work clearance
- Quarterly wellness workshops on injury prevention and posture correction
- Maintenance of confidential patient records in compliance with Saudi data protection regulations
- Monthly progress reports submitted to the Occupational Health Division

Payment for this Quotation Estimate shall be made in monthly installments of SAR 70,245.83 (inclusive of VAT), due within fifteen (15) calendar days of invoice issuance. The initial equipment and ergonomic assessment fees (Items 03 and 04) shall be invoiced separately within the first thirty (30) days of contract commencement. All payments shall be processed via bank transfer to the account designated by Al-Shifa Physiotherapy & Rehabilitation Center, Jeddah, Saudi Arabia. Late payments shall incur a penalty of 1.5% per month as stipulated under Saudi commercial law.
- This Quotation Estimate is valid for thirty (30) days from the date of issue. Beyond this period, pricing may be subject to revision based on market conditions in Saudi Arabia Jeddah.
- The Physiotherapist and all supporting staff shall comply with all applicable regulations of the Saudi Ministry of Health and the SCFHS licensing requirements.
- Al-Shifa Physiotherapy & Rehabilitation Center shall maintain professional indemnity insurance with a minimum coverage of SAR 2,000,000 for the duration of the contract.
- Either party may terminate this agreement with sixty (60) days' written notice. In the event of early termination, a pro-rata refund of unused service fees shall be issued within thirty (30) days.
- All patient records and clinical data generated during the engagement shall remain the property of the client and shall be handled in strict accordance with Saudi Arabia's Personal Data Protection Law.
- This agreement shall be governed by and construed in accordance with the laws of the Kingdom of Saudi Arabia. Any disputes shall be resolved through the competent courts in Jeddah.
- The Physiotherapist shall not be engaged in any concurrent employment or consulting arrangement that creates a conflict of interest with the client's operations in Jeddah.
- Force majeure events, including but not limited to government-mandated closures, shall suspend obligations without penalty for either party.
By signing below, both parties acknowledge that this Quotation Estimate constitutes a binding offer for the provision of Physiotherapist services as described herein, to be executed at the designated facility in Saudi Arabia Jeddah. This document, once countersigned, shall serve as the foundational agreement for the twelve-month service period.
