The Physiotherapist services outlined in this Quotation Estimate encompass a full-cycle rehabilitation programme delivered at our premises in Sri Lanka Colombo. The programme is supervised by a senior Physiotherapist holding a B.Sc. in Physiotherapy from the University of Colombo and a postgraduate diploma in Orthopaedic Rehabilitation. All sessions are conducted in a fully equipped treatment room with modern electrotherapy machines, hydrotherapy pools, and resistance training equipment. The Physiotherapist will conduct weekly progress assessments and adjust the treatment protocol accordingly to ensure optimal recovery outcomes for the client.

This Quotation Estimate is subject to the following payment terms agreed upon between the client and Wellness Physiotherapy Centre (Pvt) Ltd. in Sri Lanka Colombo:
4.1 A non-refundable deposit of 30% (LKR 169,035) of the grand total shall be paid upon acceptance of this Quotation Estimate to reserve the Physiotherapist schedule and treatment room allocation.
4.2 The remaining 70% balance (LKR 394,415) shall be payable in two equal instalments: 50% at the midpoint of the treatment programme (after session 20) and 50% upon completion of the final discharge assessment.
4.3 Payment may be made via bank transfer to our account at Commercial Bank of Ceylon, Colombo Branch, or by certified cheque. Cash payments exceeding LKR 50,000 are not accepted in accordance with Sri Lankan financial regulations.
4.4 Any additional Physiotherapist sessions required beyond the scope outlined in this Quotation Estimate shall be billed at the prevailing per-session rates and require prior written approval from the client.
4.5 Cancellation of a scheduled Physiotherapist session with less than 24 hours' notice will incur a 50% fee of the session rate. Cancellation with 24 hours or more notice will be rescheduled at no additional cost.
This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. Should the client wish to proceed with the Physiotherapist rehabilitation programme in Sri Lanka Colombo, a signed copy of this document must be returned to our office within the validity period. Upon receipt of the signed acceptance and the initial deposit, the Physiotherapist team will schedule the first assessment session within five (5) working days.
Note: All Physiotherapist services described in this Quotation Estimate are provided in compliance with the standards set by the Sri Lanka Physiotherapy Association and the Ministry of Health, Sri Lanka Colombo.
