This Quotation Estimate has been prepared specifically for the provision of comprehensive Physiotherapist services to be delivered at our facility located in Tanzania Dar es Salaam. The following document outlines the scope of treatment, associated costs, and terms governing this professional engagement.
Our licensed Physiotherapist, Mr. Emmanuel K. Mushi (Registration No. TMC/PHYS/2016/0234), will provide a structured rehabilitation programme tailored to the client's diagnosed condition of post-operative knee rehabilitation following total knee arthroplasty. The treatment plan encompasses a minimum of twelve (12) weekly sessions conducted at our clinic in Tanzania Dar es Salaam, with each session lasting approximately sixty (60) minutes.
The Physiotherapist will perform initial comprehensive assessment, develop an individualised treatment protocol, administer manual therapy, therapeutic exercise, electrotherapy, and progressive functional training. All services are rendered in accordance with the standards set by the Tanzania Medical Council and the East African Physiotherapy Association.

- A deposit of 30% (TZS 573,000) is required upon acceptance of this Quotation Estimate to secure the client's scheduled appointments with the Physiotherapist.
- The remaining balance of 70% (TZS 1,337,000) shall be payable in two equal instalments: 50% at the completion of Week 6 and 50% upon final discharge.
- Payment may be made via M-Pesa, Tigo Pesa, bank transfer to NMB Bank (Account: 0045-8821-3376), or by cheque payable to "Dar es Salaam Physiotherapy & Rehabilitation Centre."
- All prices are quoted in Tanzanian Shillings (TZS) and are inclusive of Value Added Tax (VAT) at the prevailing rate of 18% as applicable in Tanzania Dar es Salaam.
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, rates may be subject to revision.
- Any additional sessions beyond the twelve (12) included in this estimate will be billed at the standard rate of TZS 95,000 per session, subject to prior written approval.
- Cancellations must be communicated at least twenty-four (24) hours in advance. Late cancellations or no-shows will incur a fee of 50% of the session rate.
The Physiotherapist providing services under this Quotation Estimate is fully registered with the Tanzania Medical Council and carries professional indemnity insurance with AAR Insurance Limited (Policy No. AAR/PI/2025/1142). All treatment protocols are evidence-based and comply with the National Health Policy of the United Republic of Tanzania. The facility in Tanzania Dar es Salaam is equipped with modern rehabilitation apparatus including isokinetic dynamometers, ultrasound therapy units, TENS machines, and a dedicated hydrotherapy pool.
The client acknowledges that the Physiotherapist will obtain informed consent prior to any invasive or high-risk procedure. Medical records will be maintained in accordance with the Tanzania Data Protection Act, 2022, and will be retained for a minimum of seven (7) years.
