This Quotation Estimate is issued by Istanbul Physiotherapy & Rehabilitation Center to provide a detailed and transparent breakdown of all costs associated with a full physiotherapy rehabilitation programme. The services described below will be delivered by a licensed and board-certified Physiotherapist operating within the facilities of our clinic located in the Levent district of Turkey Istanbul. The complete physiotherapy rehabilitation programme outlined in this Quotation Estimate is designed to span approximately eight (8) weeks, with sessions scheduled five days per week (Monday through Friday). The client will attend the clinic in Turkey Istanbul at a mutually agreed time between 08:00 and 17:00. The assigned Physiotherapist will maintain a consistent schedule to ensure continuity of care and optimal therapeutic outcomes. In the event of a required session cancellation, a minimum of 24 hours' prior notice must be provided to the clinic to avoid a no-show fee of $50.00 per missed appointment.
The total amount of $5,514.00 USD as stated in this Quotation Estimate shall be settled according to the following schedule:
- 40% deposit ($2,205.60) — due upon acceptance of this Quotation Estimate and prior to the first session.
- 30% interim payment ($1,654.20) — due at the midpoint of the programme (end of Week 4).
- 30% final balance ($1,654.20) — due upon completion of the final discharge assessment.
Payments may be made via international bank transfer (SWIFT), credit card (Visa, Mastercard, American Express), or secure online payment portal. All transactions are processed in US Dollars. The bank details for the Istanbul Physiotherapy & Rehabilitation Center will be provided in a separate secure document upon written confirmation of this Quotation Estimate.
- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, all prices are subject to revision based on current market rates in Turkey Istanbul.
- All physiotherapy services will be performed by a qualified, licensed Physiotherapist or under the direct supervision of the lead Physiotherapist. No treatment will be administered by unqualified personnel.
- The client agrees to attend all scheduled sessions. Repeated absences (more than three unexcused sessions) may result in programme modification or early termination at the discretion of the treating Physiotherapist.
- The clinic in Turkey Istanbul is not liable for any pre-existing conditions, complications, or adverse reactions that are not directly attributable to the physiotherapy treatment provided.
- The client is advised to obtain travel and health insurance that covers physiotherapy and rehabilitation services in Turkey Istanbul prior to commencing the programme.
- All medical records, session notes, and progress reports generated during the programme are the property of the clinic. A copy of the final report will be provided to the client and, with written consent, to the referring physician.
- This Quotation Estimate does not constitute a medical diagnosis or a guarantee of specific clinical outcomes. The Physiotherapist will make reasonable efforts to achieve the best possible recovery based on the client's individual condition.
- Any disputes arising from this Quotation Estimate shall be governed by the laws of the Republic of Turkey, and the competent courts of Istanbul shall have exclusive jurisdiction.
By signing below, the client acknowledges that they have read, understood, and accepted all terms, conditions, and pricing details set forth in this Quotation Estimate for Physiotherapist services to be delivered in Turkey Istanbul.
