1. Introduction and Scope of Services

This Quotation Estimate is issued by Kampala Physiotherapy & Rehabilitation Centre to M/s. Nakivubo Industrial Park Ltd. for the provision of comprehensive Physiotherapist services within the Uganda Kampala metropolitan area. Our team of certified and licensed Physiotherapist professionals is committed to delivering high-quality musculoskeletal rehabilitation, sports injury management, post-operative recovery programmes, and workplace ergonomic assessments tailored to the specific needs of your workforce.

All services outlined in this Quotation Estimate will be delivered by a qualified Physiotherapist registered with the Uganda Physiotherapy Association (UPA) and the Uganda Registration Bureau. The Physiotherapist assigned to your account will conduct all sessions at your facility in the Uganda Kampala industrial zone or at our central clinic located along Kampala Road, depending on the nature of the treatment required.

3. Terms and Conditions of This Quotation Estimate

This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. Beyond this period, prices may be revised to reflect changes in operational costs within the Uganda Kampala healthcare sector.
All Physiotherapist services will be performed in strict compliance with the Uganda Physiotherapy Association code of conduct and the National Health Act, 2023.
Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, with the remaining 50% payable in two equal monthly instalments over the six-month service period.
The assigned Physiotherapist shall maintain full professional indemnity insurance and shall provide a copy of their UPA registration certificate upon request.
Any additional services beyond the scope defined in this Quotation Estimate will be subject to a separate written amendment and mutual agreement.
All patient records and treatment documentation generated by the Physiotherapist shall remain the property of M/s. Nakivubo Industrial Park Ltd. and shall be handled in accordance with Uganda's Data Protection and Privacy Act, 2019.
Force majeure events, including but not limited to national public health emergencies in Uganda Kampala, may result in temporary suspension of in-person services, with virtual consultations offered as an alternative at no additional cost.

4. Acceptance and Authorisation

By signing below, the client acknowledges receipt of this Quotation Estimate and agrees to the terms outlined herein for the engagement of our Physiotherapist services in Uganda Kampala. This document does not constitute a binding contract until a formal service agreement is executed by both parties.
