  1. This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiry date, pricing may be subject to revision based on prevailing exchange rates and operational costs in Zimbabwe Harare.
  2. All Physiotherapist services will be delivered by HPCZ-registered practitioners with a minimum of five (5) years of clinical experience in orthopaedic and sports rehabilitation.
  3. Payment terms: 30% advance deposit upon acceptance of this Quotation Estimate, with the remaining 70% payable in equal monthly instalments over the twelve-month service period.
  4. Payments shall be made in United States Dollars (USD) via bank transfer to our account at Stanbic Bank Zimbabwe, Harare branch. Alternative payment in Zimbabwean Dollars (ZWL) at the prevailing interbank rate on the day of payment is acceptable.
  5. The client agrees to provide a suitable, well-ventilated space at their Borrowdale facility for on-site Physiotherapist sessions. Our centre will supply all necessary equipment, treatment tables, and electrotherapy devices.
  6. Any additional sessions beyond the scope outlined in this Quotation Estimate will be billed at the standard rate of USD 65.00 per 45-minute Physiotherapist consultation.
  7. Cancellation of the contract by either party requires thirty (30) days written notice. In the event of early termination, the client shall be liable for all services rendered up to the date of termination.
  8. All patient records and clinical data will be handled in strict accordance with the Zimbabwe Data Protection Act (Chapter 24:08) and the HPCZ Code of Professional Conduct.
  9. This Quotation Estimate does not constitute a guarantee of specific clinical outcomes. The Physiotherapist will exercise professional clinical judgement in determining the most appropriate treatment pathway for each individual.
  10. Disputes arising from this agreement shall be resolved through mediation in Harare, Zimbabwe, in accordance with the laws of the Republic of Zimbabwe.

By signing below, the client acknowledges and accepts the terms, conditions, and pricing outlined in this Quotation Estimate for the Physiotherapist services to be provided in Zimbabwe Harare. The Harare Physiotherapy & Rehabilitation Centre confirms its commitment to delivering the services as described herein.
