1. Purpose of This Quotation Estimate

This Quotation Estimate is issued by MindCare Psychiatric Clinic, a registered and licensed psychiatric practice operating in Bangladesh Dhaka, to provide a comprehensive and transparent cost breakdown for professional Psychiatrist consultation and treatment services. This document serves as a formal estimate for the client to review, approve, and authorize the delivery of psychiatric care services as outlined below. All pricing reflects the current market rates for psychiatric services in the Bangladesh Dhaka metropolitan area as of June 2025.

2. Scope of Psychiatrist Services

The following Psychiatrist services are included in this Quotation Estimate. Each service is delivered by a board-certified Psychiatrist registered with the Bangladesh Medical Council and experienced in treating a wide range of mental health conditions including but not limited to depression, anxiety disorders, bipolar disorder, schizophrenia, obsessive-compulsive disorder, post-traumatic stress disorder, and substance use disorders.

4. Terms and Conditions of This Quotation Estimate

This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiry date, the Psychiatrist service rates may be subject to revision based on prevailing market conditions in Bangladesh Dhaka.
All services described in this estimate shall be delivered by a qualified and licensed Psychiatrist or under the direct clinical supervision of the treating Psychiatrist at MindCare Psychiatric Clinic, Banani, Bangladesh Dhaka.
Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, and the remaining 50% to be settled within seven (7) working days of the completion of the treatment cycle.
Accepted payment methods include bank transfer (any commercial bank in Bangladesh), mobile banking (bKash, Nagad, Rocket), and cash at the clinic reception in Bangladesh Dhaka.
The treating Psychiatrist reserves the right to modify the treatment plan based on clinical judgment. Any additional services beyond the scope of this estimate will be communicated to the client in writing prior to delivery and will be billed separately.
All patient information and psychiatric records are strictly confidential and protected under the Bangladesh Medical Council's Code of Ethics and applicable data privacy regulations.
This Quotation Estimate does not constitute a guarantee of specific treatment outcomes. The Psychiatrist will exercise the highest standard of clinical care consistent with accepted psychiatric practice in Bangladesh Dhaka and internationally recognized guidelines.
In the event of force majeure, clinic closure due to government directives, or unforeseen circumstances, the Psychiatrist will reschedule affected appointments at no additional cost to the client.
Any disputes arising from this Quotation Estimate shall be resolved amicably or, failing that, through the appropriate legal jurisdiction of Dhaka, Bangladesh.

5. Acceptance and Authorization

By signing below, the client acknowledges receipt of this Quotation Estimate for Psychiatrist services in Bangladesh Dhaka and authorizes MindCare Psychiatric Clinic to proceed with the outlined treatment plan upon the terms stated herein.
