- A non-refundable deposit of 25% of the total Quotation Estimate amount (₦890,906.25) is due upon acceptance to secure the Psychiatrist's schedule and commence services.
- The remaining balance shall be payable in three (3) equal quarterly instalments of ₦890,906.25 each, due on the 1st of January, April, July, and October 2025.
- Payment shall be made via bank transfer to the account details provided in the accompanying invoice. Cheques and cash payments are not accepted for amounts exceeding ₦50,000.
- A late payment penalty of 2% per month shall apply to any overdue instalment after a 14-day grace period.
- All fees are exclusive of out-of-pocket costs for third-party laboratory tests, imaging, or inpatient hospitalisation at the National Hospital Abuja or other facilities, which will be billed separately with prior written authorisation.
- This Quotation Estimate does not constitute a contract until countersigned by both parties. A formal Service Agreement will be executed upon acceptance.
- The Psychiatrist reserves the right to refer the patient to a specialist or inpatient facility in Nigeria Abuja if the clinical condition warrants a higher level of care, at no additional administrative cost to the client.
- All patient records, clinical notes, and progress reports are the confidential property of the provider and are protected under the National Health Act 2014 and the Data Protection Regulation (NDPR) 2019 of Nigeria.
- Either party may terminate the engagement with a written notice of thirty (30) days. In the event of early termination, the client shall be liable for all services rendered up to the termination date.
- The provider shall maintain professional indemnity insurance with a minimum coverage of ₦50,000,000, as required for psychiatric practice in Nigeria Abuja.
- Disputes arising from this Quotation Estimate or the subsequent Service Agreement shall be resolved through mediation in Abuja, FCT, Nigeria, in accordance with the Arbitration and Conciliation Act (Cap A18, LFN 2004).
