This Quotation Estimate is issued by Dra. Valentina Ríos Mendoza, a licensed Psychiatrist registered with the Colegio de Psicólogos del Perú and the Colegio Médico del Perú, to provide comprehensive psychiatric evaluation and ongoing mental health services to the employees and management of Grupo Empresarial Andino S.A.C. All services will be delivered in the Peru Lima metropolitan area, specifically at the client's corporate offices in San Isidro or at the Psychiatrist's private clinic located in Miraflores, Lima, as mutually agreed upon for each session.

The scope encompasses initial psychiatric assessments, diagnostic evaluations, therapeutic follow-up sessions, medication management (where clinically indicated and in accordance with Peruvian health regulations), and the issuance of official psychiatric reports for occupational health purposes. All clinical documentation will be maintained in compliance with the Peruvian Data Protection Law (Ley Nº 29733) and the ethical codes governing psychiatric practice in Peru Lima.

• This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, pricing may be subject to revision based on market conditions in Peru Lima.
• All services rendered by the Psychiatrist are strictly confidential. No clinical information will be disclosed to third parties without the written, informed consent of the patient, in full compliance with the Peruvian Code of Medical Ethics and Ley Nº 29733.
• Payment terms: 50% upon signing of the service agreement; 50% upon completion of the contracted service period. Invoicing will be issued in Peruvian Soles (S/) with the applicable IGV as required by SUNAT regulations.
• The Psychiatrist reserves the right to refer patients to specialized psychiatric hospitals or allied mental health professionals within the Peru Lima network if the clinical presentation exceeds the scope of outpatient care.
• Sessions are scheduled Monday through Friday, 08:00 to 18:00 hours, at the agreed-upon location in Lima. After-hours emergency consultations are available by prior phone arrangement.
• Any cancellation of a scheduled session must be communicated at least 24 hours in advance. Cancellations within 24 hours will be billed at 50% of the session fee.
• This Quotation Estimate does not constitute a binding contract. A formal service agreement must be executed by both parties before any services commence.
• All clinical records and reports will be retained for a minimum of ten (10) years in accordance with Peruvian health record-keeping regulations.

The undersigned Psychiatrist holds a Doctorate in Medicine and Surgery from the Universidad Nacional Mayor de San Marcos, Lima, Peru, and a specialized residency in Psychiatry from the Hospital Nacional Arzobispo Loayza.
