This document serves as a formal Quotation Estimate for the full spectrum of psychiatric consultations, diagnostic assessments, therapeutic interventions, and ongoing mental health management that our team of licensed Psychiatrist specialists will provide to the client identified above. All services described in this Quotation Estimate are to be rendered in compliance with the Federal Law of the Russian Federation No. 323-FZ "On the Basics of Health Protection of Citizens in the Russian Federation" and all applicable regulations governing psychiatric practice in Russia Moscow.

All prices are quoted in Russian Rubles (RUB). This Quotation Estimate assumes standard outpatient Psychiatrist services. Inpatient psychiatric hospitalization, if clinically indicated by the treating Psychiatrist, will be subject to a separate Quotation Estimate and prior written approval.

1. This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issue. After the expiration date, all prices and availability of the Psychiatrist specialists listed herein are subject to revision.
2. Payment for services outlined in this Quotation Estimate is due within fourteen (14) business days of the invoice date. Accepted payment methods include bank transfer to the clinic's account in Russia Moscow, cash at the clinic reception, and major credit/debit cards.
3. A 50% advance payment is required to secure the initial Psychiatrist consultation appointment. The remaining balance is payable upon completion of the first treatment cycle.
4. All Psychiatrist services will be conducted at the Capital Mind Health Clinic premises located in central Russia Moscow, unless a home-visit or telemedicine format has been explicitly agreed upon in writing.
5. Client medical records and psychiatric data are protected under Russian Federal Law No. 152-FZ "On Personal Data." No information will be disclosed to third parties without the client's written consent, except where mandated by Russian law.
6. Cancellation of a scheduled Psychiatrist appointment must be made no fewer than 24 hours in advance. Late cancellations or no-shows will incur a fee of 50% of the session rate.
7. This Quotation Estimate does not constitute a binding contract until both parties have signed and returned a copy. The Quotation Estimate is a good-faith estimate of costs and may be adjusted if the Psychiatrist determines that the clinical picture requires additional or modified interventions.
8. The treating Psychiatrist reserves the right to refer the client to a specialist psychiatrist, neurologist, or other medical professional within Russia Moscow if the clinical condition warrants it. Referral costs are not included in this Quotation Estimate.
9. Disputes arising from this Quotation Estimate shall be resolved in accordance with the civil legislation of the Russian Federation and under the jurisdiction of the Arbitration Court of the City of Moscow.

By signing below, the client acknowledges receipt of this Quotation Estimate for Psychiatrist services in Russia Moscow and agrees to the terms, pricing, and conditions set forth in this document. The client confirms that all information provided to the Psychiatrist team will be accurate and complete.
