3.1 Validity: This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After this period, prices may be adjusted to reflect changes in the Brazilian market, inflation indices (IPCA), or modifications in the Psychologist's professional availability.
3.2 Payment Terms: Payment shall be made via bank transfer (TED/PIX) to the account of Clinica Bem-Estar Psicológico, Banco do Brasil, Agência 1234-5, Conta Corrente 67890-1, CNPJ 12.345.678/0001-90. Invoices (Notas Fiscais de Serviço) shall be issued monthly in accordance with the municipal tax regulations of São Paulo (ISS — Imposto Sobre Serviços). Payment is due within fifteen (15) business days of invoice receipt.
3.3 Professional Registration: The Psychologist providing all services under this Quotation Estimate holds active registration with the Conselho Federal de Psicologia (CFP) and the Conselho Regional de Psicologia — 6ª Região (São Paulo), under CRP 06/123456. All services are delivered in full compliance with Brazilian professional ethics and legal standards.
3.4 Confidentiality and Data Protection: All psychological records, session notes, and personal data of employees shall be handled in strict accordance with the Lei Geral de Proteção de Dados (LGPD) and the Código de Ética do Psicólogo. No individual patient information shall be disclosed to the client organization without written, informed consent from the individual.
3.5 Cancellation and Termination: Either party may terminate this agreement with thirty (30) days prior written notice. In the event of early termination, the client shall be responsible for all services rendered up to the termination date. No refund shall be issued for sessions already conducted.
3.6 Service Location: All in-person services shall be conducted at the Psychologist's registered office located in the Bela Vista district of São Paulo, SP, or at the client's premises in São Paulo upon prior arrangement. Telepsychology sessions shall be conducted via encrypted video conferencing platforms compliant with Brazilian data protection regulations.
3.7 Applicable Law: This Quotation Estimate and any resulting contract shall be governed by the laws of the Federative Republic of Brazil, with jurisdiction vested in the courts of the city of São Paulo, SP.
By signing below, both parties acknowledge that this Quotation Estimate represents a formal proposal for the engagement of the Psychologist's professional services and agree to the terms outlined herein. This document does not constitute a binding contract until a formal service agreement (Contrato de Prestação de Serviços) is executed by both parties.
