- This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, the Psychologist reserves the right to revise the rates and terms presented herein.
- All services will be rendered by Dr. Élise Tremblay or, where necessary, by a qualified associate Psychologist under her direct supervision, in accordance with the professional standards of the Ordre des psychologues du Québec.
- Payment terms: 50% of the total Quotation Estimate amount is due upon acceptance of this document. The remaining 50% shall be invoiced in two equal monthly installments over the service period. All payments are due within thirty (30) days of invoice date.
- Confidentiality: All psychological records, assessment data, and session notes are protected under the Personal Health Information Protection Act (PHIPA) of Quebec and the federal Privacy Act of Canada. No information will be disclosed to the client organization without the written, informed consent of the individual client, except as required by law.
- The Psychologist maintains professional liability insurance with a minimum coverage of $5,000,000, as required for the practice of psychology in the Province of Quebec, Canada.
- Any additional services beyond the scope described in this Quotation Estimate will be subject to a separate written amendment and additional Quotation Estimate prior to commencement.
- All sessions will take place at the Psychologist's office located in downtown Montreal, Quebec, Canada, or at the client's premises in Montreal as mutually agreed upon. Travel within the Montreal metropolitan area is included; travel outside the Montreal region will be billed at $0.85 per kilometre.
- This Quotation Estimate does not constitute a guarantee of specific therapeutic outcomes. The Psychologist commits to providing evidence-based, ethically sound psychological services in accordance with the Canadian Psychological Association's Code of Ethics.
- Disputes arising from this Quotation Estimate shall be resolved through mediation in Montreal, Quebec, Canada, in accordance with the Civil Code of Quebec.
