The Psychologist identified in this Quotation Estimate holds a professional degree in Psychology from the Universidad de Chile, a Master's degree in Clinical Psychology from the Pontificia Universidad Católica de Chile, and maintains active registration with the national professional registry. All services are rendered in the professional office located in the Providencia commune of Santiago, Chile, or alternatively via secure telehealth platforms as specified in the service schedule below.

Payment for the services outlined in this Quotation Estimate shall be made in Chilean Pesos (CLP) via bank transfer to the account designated by the Psychologist, or by certified cheque payable to the registered professional name. The client agrees to the following payment schedule:

• Initial Assessment (Item 1): Due in full prior to the first session.
• Weekly Therapy Sessions (Item 2): Payable in advance, no later than 48 hours before each scheduled session. A 15% late fee applies to payments received after the due date.
• Psychological Testing (Item 3) and Clinical Report (Item 4): Payable in two equal installments — 50% upon scheduling and 50% upon delivery of the final report.

This Quotation Estimate does not constitute a guarantee of specific therapeutic outcomes. Any additional services beyond those itemized above will be subject to a supplementary Quotation Estimate issued prior to commencement.

This Quotation Estimate is valid for a period of 30 calendar days from the date of issue. Upon acceptance, the client shall sign and return a copy of this document, which will then serve as the binding service agreement between the parties. The Psychologist reserves the right to decline or modify services if the client's clinical presentation falls outside the scope of the Psychologist's professional competence, in which case a referral to a specialist colleague in Santiago will be provided at no additional cost.
