The Psychologist assigned to this engagement holds a Doctorate in Clinical Psychology from Alexandria University and is registered with the Egyptian Psychologists Syndicate under license number EGY-PSY-2024-0871.

The lead Psychologist for this engagement, Dr. Nour El-Sayed, holds the following qualifications relevant to this Quotation Estimate:

• Ph.D. in Clinical Psychology, Faculty of Psychology, Alexandria University (2016)

• Licensed Clinical Psychologist, Egyptian Psychologists Syndicate (License EGY-PSY-2024-0871)

All in-person Psychologist sessions referenced in this Quotation Estimate will be conducted at the Alexandria Mind & Wellness Center premises located on Corniche El-Nil Street, Egypt Alexandria, or at the client's designated facility within the Alexandria governorate. The Psychologist will travel to corporate sites within Alexandria at no additional charge for locations within a 15-kilometer radius of the central office. Sessions are available Monday through Saturday, 9:00 AM to 8:00 PM, aligned with local business hours in Egypt Alexandria.

• A 30% advance payment of the total Quotation Estimate amount (EGP 18,330) is due upon acceptance of this document.

• The remaining 70% shall be invoiced in monthly installments aligned with the service delivery schedule.

• Payment may be made via bank transfer to Alexandria National Bank, Account No. 4471-2209-8834, or by certified check payable to Alexandria Mind & Wellness Center.

• A 2% late payment surcharge applies to any invoice outstanding beyond 14 days of the due date.

6.1 This Quotation Estimate is valid for thirty (30) calendar days from the date of issue. After the expiration date, the Psychologist service rates may be revised to reflect current market conditions in Egypt Alexandria.

6.2 The Psychologist reserves the right to reschedule sessions with a minimum of 24 hours' written notice. Cancellations by the client with less than 24 hours' notice will be billed at 50% of the session fee.

6.3 All psychological records, assessment results, and treatment documentation remain the confidential property of the client and are protected under Egyptian Law No. 10 of 2018 on Personal Data Protection.

6.4 This Quotation Estimate does not constitute a binding contract until countersigned by both parties. The Psychologist engagement shall be governed by a separate Service Agreement executed upon acceptance.

6.5 Any disputes arising from this Quotation Estimate or the subsequent Psychologist services shall be resolved through the Alexandria Commercial Court in accordance with Egyptian law.

By signing below, the client acknowledges receipt of this Quotation Estimate for Psychologist services in Egypt Alexandria and agrees to the terms, pricing, and conditions outlined herein. This acceptance authorizes Alexandria Mind & Wellness Center to commence the outlined services upon receipt of the advance payment.
