All Psychologist professionals engaged under this estimate hold valid registration with the Indonesian Psychological Association (HIMPSI) and are licensed to practice in accordance with the regulations set forth by the Ministry of Health of the Republic of Indonesia.

1. This Quotation Estimate is valid for a period of thirty (30) calendar days from the date of issuance. Any request for services beyond this validity period will require a revised estimate reflecting current Psychologist professional rates in Indonesia Jakarta.
2. All Psychologist services will be conducted in strict compliance with the Indonesian Code of Ethics for Psychologists (Kode Etik Psikologi Indonesia) and applicable data protection regulations under Law No. 27 of 2022 concerning Personal Data Protection.
3. Payment terms: 50% advance payment upon acceptance of this Quotation Estimate, and 50% upon completion of all services. Payment shall be made via bank transfer to the account designated by Psyche Wellness Center Jakarta.
4. The Psychologist team assigned to this engagement will be based in Jakarta and will conduct all in-person sessions at the client's premises located in South Jakarta, unless otherwise agreed in writing.
5. Confidentiality: All psychological records, assessment results, and session notes are the exclusive property of the client and the individual employee. No information will be disclosed to third parties without written consent, in accordance with professional Psychologist ethical standards in Indonesia.
6. Any additional services beyond the scope described in this Quotation Estimate will be subject to a separate written amendment and additional cost approval.
7. This Quotation Estimate does not constitute a binding contract until formally accepted in writing by both parties and a Service Agreement is executed.
8. All services are subject to the availability of the assigned Psychologist. In the event of unavailability, a qualified substitute Psychologist of equivalent credentials will be provided at no additional cost.

By signing below, the client acknowledges receipt of this Quotation Estimate for Psychologist services in Indonesia Jakarta and agrees to the terms and conditions stated herein. This document serves as a formal cost proposal and does not guarantee service commencement until a separate Service Agreement is signed.
